Why Did the Sydney Seaplane Crash on New Years Eve 2018?

On New Years Eve 2018, Richard Cousins, his fiancee Emma Bowden, children and pilot all died in a Sydney Seaplane crash into the Hawkesbury River.  I asked the tarot cards:
 'Why did the plane crash?' As up an till now it has been a bit of a mystery as why the crash occurred and an investigation is still ongoing.
This tarot reading performed on the night of the 20th of January 2018:

1. My cards show me the 'Surgeon' and the 'Butterfly of Hope' - this symbolises an autopsy of some kind is somehow revealing about how the crash occurred
   
2. The 'Bicycle pedal'  symbolises quite potentially something here, the action the pilot makes to do with his feet as a key aspect as to why the crash occurred, referred to as the 'rudder' on a plane. Perhaps it was malfunctioning. 
3. The 'Microchip/Electronics' card was also paired with 'The Sky' perhaps some wiring here was playing up, maybe a gadget was not reflecting the degrees/axis of the horizon in someway accurately.
4. The 'Attraction' card and the 'Bicycle Pedal' were paired together. It is an unusual combination, the pilot seemed to have been distracted, or was attracted or drawn to someone on the plane. The only other person that came out in this reading, was 'The Blond' as a key person of influence on this flight. Nobody else came out except her and the pilot.
5. 'The Male Actor' and 'Deceit, gossip and lies' paired together, is an indication of a major argument on the plane between passengers, or someone said something that caused a bit of a stir, perhaps this was also a distraction for the pilot. The pilot wasn't fully concentrating. This is a very unusual combination, it can mean other things..
6. 'Parliament House' was paired with 'The Helicopter' - we see government forces taking extra action around this disaster
7. The most significant organ of reference here to do with the human body, was the 'Lungs & Heart' which was the only bodily organ coming out in this reading
8. Lastly we see 'The Classroom of Learning' paired with the 'Ocean of Prosperity' - those working on this disaster, whom are placing the pieces together, will learn and it will be solved, I see information being presented in a classroom like environment, like a presentation or lecture of some kind.
